Scalable and Explainable Patient-Centric Healthcare Blockchain Using Deterministic Sharding and Quantum-Aware Optimization

Abstract
The patient-centered medical system demands the presence of digital infrastructures, which are scalable, secure and have high standards of strict regulations. Even though EHR management can be decentralized and audit-able with blockchain technology, the existing healthcare blockchain solutions have throughput and latency limitations in addition to poor explainability in systems and security issues that emerge post-quantum. This paper introduces a quantum resilient, explainable sharded blockchain architecture of scalable healthcare systems of the patient. The paper incorporates deterministic sharding to support parallel execution of transaction with deterministic data locality, role-based access control (RBAC) smart contracts to support contrivance interpretable and auditable access policies as well as circuit optimization using ZX calculus. Explainability Coherency is brought together with the disclosure of explicit shard allocation, access-control decision and execution metrics. The proposed framework is implemented on a test Ethereum-based network and with healthcare transaction loads using the Multi-Machine Blockchain Performance Dataset. The experimental results demonstrate that it is better in terms of higher throughput, lower transaction latency, and lower execution cost as compared to non-sharded and non-explainable baselines model as well as lower complexity through quantum-aware optimization to produce a measurable circuit.
